To Book an Appointment.
We require a requisition from your healthcare provider in order to book an appointment. Generally, one is sent in on your behalf, however you are able to book the appointment yourself if you have the requisition in hand. We kindly ask that you bring the requisition with you once you have booked in for testing.

*Patients less than 15 years of age should be referred to Paediatric Respirology / Alberta Children’s Hospital
